About the Registration Form
What the Rainbow DJ Lab interest list asks today, what full registration will ask after launch, why we need each item, and what happens after you submit.
Where the form is
The online form is at /dj-lab/enroll/register.html. This page explains it so you know what to expect before you open it.
Please note: no class dates are set yet, and the program still needs Chicago Park District approval. Until every item on the launch checklist is complete, the online form is an interest list, not a registration. See the enrollment overview.
Today: the interest list
The interest list lets us contact you when registration opens. It takes a few minutes and asks only:
| Item | Why we ask |
|---|---|
| Child's first name and last initial | So we know who the interest is for. |
| Age and age group | So we know which groups families are interested in. A 9-year-old goes in Junior DJs (ages 8–11). |
| Guardian name, email and phone | So we can contact you when dates are set and full registration opens. |
| Permission to contact (required) | You confirm you're the child's parent or legal guardian, that Ras Tafari Inc. may contact you about the Rainbow DJ Lab, and that you understand this is an interest list, not enrollment. |
What the interest list does NOT collect: health or allergy information, pickup adults, custody details, emergency contacts, consents, or free-text notes.
What it means:
- It is not enrollment and doesn't hold a place in a class. An interest-list entry can never be approved onto a class; every family completes full registration first.
- If the Lab hasn't started within 12 months, we delete the list. You can ask us to remove your entry sooner at any time.
The printable PDF forms are for full registration after launch. You don't need them for the interest list.
After launch: full registration
Full registration opens only when every launch-checklist item is complete. Plan on 15–20 minutes. Before you start, have these handy:
- Phone numbers for two emergency contacts
- Names, relationships and phone numbers for every adult who may pick up your child
- Your child's allergy and medication details, if any
- A copy of any custody or court order we must follow
What full registration asks, and why
| Item | Why we need it |
|---|---|
| Child's first name and last initial | This is how your child shows up on the in-room station board. We don't put full names on any screen. |
| Age and age group | To place your child in the right group. A 9-year-old goes in Junior DJs (ages 8–11). |
| Accommodations | So we can set up the room, the pacing and the jobs to fit your child (for example, sensory needs, a quieter spot, or extra time). |
| Medical and allergy information | So the adults in the room know what to watch for and what to do. See medical and allergy. |
| Guardian name, email and phone | So we can reach you the same day about anything that happens, send your confirmation, email your private family link, and email you at every sign-out. |
| Two emergency contacts | In case we can't reach you. See emergency contacts. |
| Authorized pickup adults (name, relationship, phone) | Children are released only to adults on this list, after a photo ID check. See authorized pickup. |
| May your child sign themselves out? | The default is no. Children 8–11 are never released to walk home alone unless a parent or guardian gives written permission in advance. |
| Custody note (optional) | Any custody or court order we must follow, such as an adult who may not pick up. Please bring a copy of the order. |
| Media consent (optional) | Whether we may photograph or record your child for program use. Saying no has no effect on your child's place. See media consent. |
| Performance consent (optional) | Whether your child may perform for an audience. Without it, your child is never put in the Spotlight on the main speaker. See performance consent. |
| Digital tracking consent (optional) | Whether your child's in-class activity is saved in the class app. Without it, nothing your child does in the app is saved; they tell the coach their answers instead. See digital tracking consent. |
| Recap emails (optional) | A short recap after each class. |
| Code of conduct agreement (required) | You and your child agree to the same simple rules. See code of conduct. |
| Guardian permission box (required) | You confirm you're the child's parent or legal guardian and give permission for them to take part. For children under 13, this is also the verifiable parental consent that federal children's privacy law (COPPA) requires. |
Each consent is asked separately, so you can say yes to some and no to others.
Both forms use Cloudflare Turnstile, a quick bot check, to keep spam out.
Who sees your answers
Your contact details, emergency contacts, medical and allergy notes, pickup list and consents are stored privately for coaches only and are never sent to a family link or an in-room screen.
- Your private family link shows only your own child.
- In-room Kid/Station screens use a room code the coach issues for each class, which expires. They show each child's first name and last initial, station, job and in-class activity, never private details or coach notes.
Read more on the privacy page.
We use Supabase to store data and Resend to send email. We don't run ads, use tracking pixels, or sell or share your data.
How long we keep it
- Interest-list entries are deleted if the Lab hasn't started within 12 months, or sooner if you ask.
- Registration records are kept for 3 years for safety and insurance purposes, then deleted.
- Child activity data from the app is deleted within 90 days after the season ends, unless you ask us to keep your child's Passport.
These are policy commitments. You may ask to review, correct or delete your information at any time.
What happens after you submit
The interest list: you'll get an email confirming you're on the list and that it is not enrollment. When the program is approved and every launch-checklist item is complete, we'll invite you to finish full registration. Want off the list? Reply to that email or call Stephen.
Full registration (after launch):
- You'll get a confirmation email right away. If it doesn't come, check your spam folder, then contact Stephen.
- If you listed allergies, medication or accommodations, Stephen Henry will contact you to go over the plan and any paperwork, such as a medication authorization or a FARE emergency care plan.
- When a coach approves your child for a class, you'll get your private family link by email, plus the schedule and location details.
- On the first day, arrive 10 minutes early to sign your child in.
Need to change something?
Contact Stephen Henry at 872-255-9333 or stephen@selassiefest.com. Changes to your pickup list must be made in writing (email is fine) before the class where they apply.
