Emergency Contacts Form
Two adults we call if something happens and we can't reach you.
Purpose
If your child is hurt, gets sick, or needs to be picked up and we can't reach you, we call these adults. In a medical emergency we call 911 first, then you, then your emergency contacts.
Fill it out online: part of full registration, which opens only after every launch-checklist item is complete. Until then the online form is an interest list that doesn't ask for this. Printable version (for full registration after launch): emergency-contacts.pdf
Every field
You'll fill this out for two contacts, both required:
| Field | Why |
|---|---|
| Name | So staff know who they're speaking with. |
| Relationship to child | Helps staff and your child (for example, "Aunt," "Neighbor," "Grandfather"). |
| Phone | The number we call. A mobile number that can receive texts is best. |
Being an emergency contact does not by itself allow pickup. Only adults on your authorized pickup list may take your child, and they must show photo ID. If you want an emergency contact to be able to pick up, list them on both.
Choose adults who are usually reachable during class time, and let them know you've listed them.
Who sees it
Coaches and Stephen Henry only. It's stored privately and never shown on the class page or in the class app's family view.
How long it's kept
It's kept as part of the registration record for 3 years for safety and insurance purposes, then deleted (policy commitment).
