Media Consent

Optional permission for Ras Tafari Inc. to photograph or record your child, which you can take back at any time.

Purpose

This optional form asks whether we may take photos, video or audio recordings of your child during Lab activities and use them to share what the program does. The default is no. Saying no never affects your child's place, their jobs at the station, or their chance to perform.

Please note: the class app does not store photos or video. This consent is about photos and recordings that Ras Tafari Inc. takes for its own program materials. In-class listen-back recordings (2-minute practice mixes that children review with a mentor) are learning tools, not media for publication, and they're covered here only if they're shared outside class.

Media Consent: Rainbow DJ Lab, Ras Tafari Inc.

I am the parent or legal guardian of the child named on this registration. By checking the boxes above, I give Ras Tafari Inc. permission to photograph, video record and/or audio record my child during Rainbow DJ Lab activities, and to use those images and recordings, without payment, to describe and promote the Rainbow DJ Lab, Full Spectrum at Rainbow Beach, and the nonprofit work of Ras Tafari Inc. This may include its websites, social media, printed materials, reports to funders, and presentations to the Chicago Park District.

Ras Tafari Inc. agrees that: 1. It will never publish my child's last name, school, home address, or any other information that would help a stranger locate my child. 2. It will use my child's first name only if I checked that box. 3. It will not sell images or recordings of my child or give them to anyone for advertising. 4. It will not use images in a way that would embarrass my child, and it will remove any image I or my child object to. 5. It will not use facial recognition or any biometric processing on images of my child.

I understand this consent is optional and that refusing it will not affect my child's participation.

I understand I may withdraw this consent at any time by contacting Stephen Henry (872-255-9333, stephen@selassiefest.com). After I withdraw, Ras Tafari Inc. will make no new uses of my child's images or recordings and will remove them from websites and social media accounts it controls. Materials already printed or distributed before my request may not be recallable.

This consent lasts until I withdraw it or until the end of the season in which it was given, whichever comes first. It must be renewed each season.

Photos by other families

At the Rainbow Showcase and public events, other families may take their own photos. We'll ask everyone to photograph only their own child. If your child must not be photographed for safety reasons, tell Stephen privately, and we'll help with seating and positioning.

Who sees it

Your consent choices are stored privately for coaches and Stephen Henry only, so staff know which children may be photographed.

How long it's kept

Your consent record is kept with the registration record for 3 years, then deleted (policy commitment). Published images are removed when you withdraw consent, as described above.
