Registration Form
The core enrollment form that tells us who your child is, which group they join, and how to reach you.
Purpose
This is the main enrollment record. It tells us who your child is, places them in the right age group, tells us how to support them, and tells us how to reach you. It also holds the guardian permission box that makes the enrollment official.
When it's available: full registration opens only when every item on the launch checklist is complete. The printable version, registration.pdf, is also for full registration after launch.
Until then, the online form at /dj-lab/enroll/register.html is an interest list, not this form. The interest list asks only your child's first name and last initial, age and age group; your name, email and phone; and your permission for us to contact you. It isn't enrollment, it collects no health, pickup, custody, emergency-contact or consent information and no free-text notes, and the list is deleted if the Lab hasn't started within 12 months. An interest-list entry can never be approved onto a class.
Every field
| Field | Required? | Why |
|---|---|---|
| Child's first name | Required | Used by coaches and on the in-room station board. |
| Child's last initial | Required | Tells apart kids with the same first name. We don't put full last names on any screen. |
| Age | Required | Placement and safety (for example, sign-out rules for ages 8–11). |
| Age group | Required | Little DJs (5–7), Junior DJs (8–11), Teen DJs (12–17), Grown-Up & Elder DJs (18+), or Empress Decks (a girls-only track, about 9–17, welcomed by park staff; written confirmation will be part of the park agreement). |
| Accommodations | Optional | Anything that helps your child do their best: sensory needs, attention, mobility, language, a quieter spot, extra time. You don't need to share a diagnosis. |
| Guardian name | Required | The adult responsible for enrollment. |
| Guardian email | Required | Confirmation, your private family link, schedule, cancellations, an email every time your child is signed out, and recaps if you opt in. |
| Guardian phone | Required | Same-day calls about incidents, illness, weather and pickup. Texts about cancellations. |
| Code of conduct agreement | Required | See code of conduct. |
| Guardian permission box | Required | You confirm you're the child's parent or legal guardian, that the information is accurate, and that you give permission for your child to take part. For children under 13, this is the verifiable parental consent required by the federal Children's Online Privacy Protection Act (COPPA). |
Full registration also includes the emergency contacts (two), medical and allergy, authorized pickup with an optional custody note, and the optional media, performance and digital tracking consents, each asked separately.
Who sees it
- In-room station board (screens with the coach's expiring room code): only your child's first name and last initial, with their station, job and in-class activity.
- Your private family link: only your own child.
- Coaches and Stephen Henry only: everything else. It's stored privately with our database provider, Supabase, and never sent to a family link or an in-room screen.
- We use Resend to send your confirmation email. We never sell or share your information.
How long it's kept
Registration records are kept 3 years for safety and insurance purposes, then deleted (policy commitment). Interest-list entries are deleted if the Lab hasn't started within 12 months, or sooner on request. You can review, correct or ask about deletion at any time. See withdrawing and privacy.
Guardian permission statement (as it appears on the form)
I am the parent or legal guardian of the child named above. The information I've given is accurate, and I'll update it if it changes. I give permission for my child to take part in the Rainbow DJ Lab, run by Ras Tafari Inc. I understand the program is proposed, that no class will run until its launch requirements are met, and that I may withdraw my child or my consent at any time by contacting Stephen Henry.
