Every Rainbow DJ Lab form in one place, with what each is for and links to the online form and printable PDFs.
These forms are for full registration, after launch
Right now, you don't need any of these forms. Until every item on the launch checklist is complete, the online form is an interest list. It asks only your child's first name and last initial, age and age group, your name, email and phone, and your permission for us to contact you. It isn't enrollment, it collects no health, pickup, custody, emergency-contact or consent information and no free-text notes, and the list is deleted if the Lab hasn't started within 12 months.
Full registration opens only when every launch-checklist item is complete. Then all of the forms below are combined into a single online form that takes about 15–20 minutes, with each consent asked separately. Each page below explains one part of it in detail: what it asks, why, who sees it, and how long we keep it.
The printable PDFs are for full registration after launch. Please don't send us health, pickup or custody information before then. After launch, if you'd rather fill things out on paper, use the printable PDFs. Bring them to Stephen Henry, or scan or photograph them and email them to stephen@selassiefest.com. If a PDF link isn't working yet, contact Stephen and he'll get you a copy.
Spanish versions of every parent form are a launch requirement. They aren't ready yet.

Saying no to an optional form never affects your child's place in the program.
Separate forms we may ask for
Depending on your child's needs, Stephen will follow up with:
- a medication authorization for any medication your child brings, which must be in its original pharmacy container
- a Food Allergy & Anaphylaxis Emergency Care Plan (the FARE form), signed by your child's doctor, for children with serious allergies

Who sees form information
Guardian contact details, emergency contacts, medical and allergy notes, the pickup list and consents are stored privately for coaches only and are never shown on a family link or an in-room screen. If a coach adds a child from paper forms, the coach records which consents are on paper, and the app enforces them the same way. They're stored with our database provider, Supabase. See privacy.
How long we keep it
Registration records (including these forms) are kept for 3 years for safety and insurance purposes, then deleted. Interest-list entries are deleted if the Lab hasn't started within 12 months, or sooner if you ask. These are policy commitments. You can ask to review or correct your information at any time.
Updating a form
Information changes, so tell us any time. Email stephen@selassiefest.com or call or text 872-255-9333. Changes to the pickup list must be in writing (email counts) before the class where they apply.
